The client Quality of Service (QoS) survey offers valuable provider feedback and service validation for research, invoice auditing, and tracking purposes. Mental Wellness Support Program participation criteria include a requirement that all approved registrants submit a QoS survey after EVERY visit.
A QoS Survey Support Line has been established to assist those without internet and computer access to complete the survey by phone. The QoS Survey Support line can be reached at our Main Office (213) 222-6327 and select Programs from the menu.
